How to Open an AMG Account

Once you know what product best suits your client’s needs, opening an AMG Super account for your client is simple.

AMG Personal Super

You and your client need to complete and sign the following documents:

1)  To Open a AMG Account:

• Member Application Form

• Adviser Remuneration Form

• Direction and Indemnity Form – only for clients investing in Direct Shares

2)  To Transfer Money into AMG:

• Transfer Form (ensure you provide an original certified identification of your client)

3) For Insurance Cover:

• If your client is transferring existing cover within another super fund to AMG – Insurance Transfer Form

• If you client is applying for new insurance – Insurance Cover Application Form 

• If your client wishes to make a Binding Beneficiary Nomination - Binding Nominiation of Beneficiary Form

Once all forms are completed, please send to:

If your client is investing in direct shares, once we receive the application we will send the CHESS Sponsorship Form to your office so you can set up the HIN and the account for the client.

AMG Account Based Pension

You and your client need to complete and sign the following documents:

1) To Open a AMG Account:

• Pension Application Form (ensure you provide an original certified identification of your client)

• Adviser Remuneration Form

• Direction and Indemnity Form – only for clients investing in Direct Shares

 2) To Transfer Money into AMG:

• Transfer Form (ensure you provide an original certified identification of your client)

Once all forms are completed, please send to:
