Once you know what product best suits your client’s super needs, establishing a client with AMG Universal Super is simple.
AMG Personal Super – Your client needs to complete and sign the following documents:
- Member Application Form (pg A-71 of the PDS);
- Investment Choice Form (pg A-77 of the PDS); and
You will need to complete the following documents:
- Adviser Remuneration Form – Declaration and Indemnity (pg A-80 of the PDS); and
- Provide original certified identification of your client.
Please refer to the AMG Personal Australian Share Portfolio section below for clients who are investing in this investment option. Once all forms are completed, please send to the address below.
AMG Account Based Pension - Your client needs to complete and sign the following documents:
- Account Based Pension & Term Allocated Pension – Membership Form (page A-73 of the PDS)
- Application to Commence Pension (page A-75 of the PDS)
- Investment Choice Form (page A-77 of the PDS); and
You will need to complete the following documents:
- Adviser Remuneration Form – Declaration and Indemnity (pg A-80 of the PDS); and
- Provide original certified identification of your client.

AMG Personal Australian Share Portfolio – If your client is investing in the AMG Personal Australian Share Portfolio please note this in the Investment Choice Form. Furthermore, you will need to complete the:
- Direction and Indemnity Form

Once all paperwork is received, a Money Market and CHESS Sponsorship Form will be sent to your office. The Money Market Form will need to be signed by you (the financial adviser) and returned to AMG Universal Super. Linking of the Money Market account and your administration system will need to be initiated by you.
