Once you know what product best suits your super needs, joining AMG Universal Super is simple.
Step 1:
Download the Product Disclosure Statement (PDS) from here, and ensure you are happy with becoming a member of AMG Universal Super.
Step 2:
AMG Employer Sponsored members – Complete and give the following forms to your employer:
- Member Application Form (page A-73 of the PDS); and
- Investment Choice Form (page A-79 of the PDS).
AMG Personal members – Complete and send the following forms to the address below:
- Member Application Form (page A-73 of the PDS); and
- Investment Choice Form (page A-79 of the PDS).
AMG Term Allocated Pension and AMG Account Based Pension members – Complete and send the following forms to the address below:
- Account Based Pension & Term Allocated Pension – Membership Form (page A-75 of the PDS)
- Application to Commence Pension (page A-77 of the PDS)
- Investment Choice Form (page A-79 of the PDS); and
- Provide certified identification.

Step 3:
If you want to transfer your other super accounts into AMG Universal Super, also complete the:
- Transfer Form (page A-84 of the PDS) remember to enclose original certified identification as your other fund will not be able to release your super until they have this.
